luImmunisationFormStatus lookup table

Description

The luImmunisationFormStatus lookup table is used to define statuses for immunisation forms.

The lookup table is used to populate the Form Status field on the Immunisation tab of the Medical Maintenance window. See:

• Medical Maintenance - Immunisation tab in the SynWeb Medical details manual.

Example

Fields

Field	Description
Code	Unique identifier for the immunisation form status.
Description	Description of the immunisation form status.
CEOCode	Status of the immunisation form as registered with the Catholic Education Office.
ExternalSystemType	Code for the external authority that provides the ExternalSystemCodes. List of External Authorities/Data Providers: CEOVIC Overview of Reference Tables - Externally Supplied Data
ExternalSystemCode	Code for the immunisation form status used by the external system.
SynergyMeaning	Code to tell Synergetic how to treat the selected immunisation form status. See SynergyMeanings. This field currently only has one value: Complete. You can assign Complete to any immunisation form status to define the 'Immunisation Complete' status; selectable in the Immunisation tab - Form Status field to indicate student is fully immunised. See Medical Maintenance - Immunisation tab.
ModifiedDate	Date and time the lookup table entry was last modified.
ModifiedUser	User that last modified the lookup table entry.
