Add A Medical Condition
- Go to Community Portal
- Click on My Details Tab
- Click on Medical Tab
This will bring up a list of medical conditions that can be selected.
The list of available medical conditions can be edited in Synergetic Windows application > System > Lookup Table Maintenance > luMedicalConditionType.
If a medical condition that is in the lookup table is not appearing on Community Portal, ensure that the Active Flag is ticked.

- Click the Edit button
- Select the medical condition you would like to add by Clicking on the check box

- Enter the Severity of the condition (the drop down list that appears can be edited in the Synergetic Windows application > System > Lookup Table Maintenance > luMedicalConditionSeverity. If an entry on the list is not appearing ensure that the Active Flag is ticked.
- Enter any other relevant information as required in Details / Emergency Treatment Details / Other Treatment Details
- Click Save

Add Medication
- Click Add Medications

- Select a Medication from the drop down list
This list can be edited in the Synergetic Windows application > System > Lookup Table Maintenance > luMedication
- Enter any Medication Details / Dosage
- Enter when the Medication should be taken
This list can be edited in the Synergetic Windows application > System > Lookup Table Maintenance > luMedicationType
- Enter the Administration Method
This list can be edited in the Synergetic Windows application > System > Lookup Table Maintenance > luMedicationAdministration
- Enter any Medication Details / Dosage
- Click Save

Link Medication to Medical Condition
The medication you have just added will now appear in the Medications section
To link the medication to the medical condition
- Click Edit

- Click on Edit Conditions
- A pop up will appear asking you to select which Medical Condition the medication is to be Used for

- Click the checkbox next to the Medical Condition that you would like to link the Medication to
- Click Close

- Scroll down to the Medications Section
- You should see the Medical Condition you linked in the previous step appear under Medical Conditions this Medication is used for

- Click Save
- The Medication should now be linked to the Medical Condition
(Depending on the school's policy regarding changes made in Community Portal, a staff member may need to approve the changes made in Action Center.
